These are required for all pets undergoing anesthesia.  NOTE:  This form contains important information regarding the risks of anesthesia and measures available to help minimize these risks.

Cancellation Policy:  If you need to cancel your appointment, please notify us at least 24 hours in advance.  There is a $25.00 fee per pet for missing an appointment.  This fee is to encourage our clients to take their appointments as seriously as we do.  This time is reserved for your pet(s) and, if you fail to keep the appointment, we are unable to accommodate other patients.

Returned Check Fee:  There is a $25.00 fee for all returned checks.  This fee, along with the amount of the original check, is due in cash within one weekof notification by our office.
